How to make solo 401k contributions | Self-Directed 401k Contributions

QUESTION 1: Is there a form required to make solo 401k contributions? Or do I just make a deposit?

QUESTION 2: Do Employee contribution and Employer solo 401k contributions have to be made as separate deposits?

ANSWER:  Both contribution types (employee and employer) can be made in one or multiple deposits; the key is not exceed the annual contribution limits which are listed on the following page.
